Somatus CKD-ESKD Program Go-live is November 1, 2026, for Wellcare Meridian Dual Align (HMO D-SNP)

A new Somatus Chronic Kidney Disease (CKD) and End Stage Kidney Disease (ESKD) healthy living support program aims to improve outcomes and the quality of life for Wellcare Meridian Dual Align (HMO D-SNP) members. The support program for eligible members is designed to improve quality measures and clinical outcomes with a goal of more healthy days for patients.  

Somatus, a leading value-based kidney care organization, provides eligible members, with or at risk of CKD/ESKD, a personal support team of health professionals to manage their kidney disease while actively following a treatment plan, with a goal to keep patients out of the hospital, improve their quality of life, and slow or stop disease progression.

The Somatus healthy living program is available at no cost for Wellcare Meridian Dual Align (HMO D-SNP) members. To locate patients in your practice that are eligible, please email provider@somatus.com.

• One-to-one care to help manage kidney disease and comorbidities and address social determinants of health
• Personal health coaching based on conditions, treatment options, and diet
• Assistance to transition safely from hospital to home
