Long-Term Services and Supports

Long-Term Services and Supports (LTSS) encompass a wide range of home- and community-based services that help members live safely and independently in their own homes or community settings. In Michigan, Meridian Health Plan partners with Michigan Department of Health and Human Services (MDHHS) to coordinate LTSS for members who have chronic health conditions, functional limitations, or other long-term care needs. Our goal is to ensure members receive the right care, in the right place, at the right time.

LTSS services are available to eligible members who may require assistance with daily activities or ongoing medical support. These services are designed to help members remain in the least restrictive, most supportive environment possible. LTSS is provided to members who have been approved to receive additional support under the Home & Community-Based Services (HCBS) waiver in the MI Coordinated Health (MICH) program (the state’s new HIDE DSNP Product).
