Long-Term Services and Supports
Long-Term Services and Supports (LTSS) encompass a wide range of home- and community-based services that help members live safely and independently in their own homes or community settings. In Michigan, Meridian Health Plan partners with Michigan Department of Health and Human Services (MDHHS) to coordinate LTSS for members who have chronic health conditions, functional limitations, or other long-term care needs.
Our goal is to ensure members receive the right care, in the right place, at the right time. LTSS services are available to eligible members who may require assistance with daily activities or ongoing medical support. These services are designed to help members remain in the least restrictive, most supportive environment possible. LTSS is provided to members who have been approved to receive additional support under the Home & Community-Based Services (HCBS) waiver in the MI Coordinated Health (MICH) program (the state’s new HIDE DSNP Product).

What do you enjoy most about working with providers?
I enjoy building strong, collaborative relationships with providers and serving as a trusted resource to support their success. It is rewarding to help providers understand processes, navigate challenges, and ensure they have the tools and information needed to deliver quality care to members. I also value the opportunity to identify recurring issues and provide education that improves efficiency and reduces future barriers.
What motivates you in your work?
I am motivated by the opportunity to make processes more efficient and impactful for both providers and members. Seeing positive outcomes such as reduced escalations, improved compliance, and increased access to services drives my work. I am also motivated by continuous improvement, using feedback and data to identify root causes, implement solutions, and contribute to operational excellence while supporting strong, trusted partnerships.

What do you enjoy most about working with providers?
What I enjoy most about working with providers is building strong, collaborative partnerships. I value taking the time to get to know providers on a personal level and truly understand the impactful work they do every day. They are the experts on the front lines, caring for some of our most vulnerable members, and their insight is invaluable. My goal is to support their success by removing roadblocks and making processes easier so they can focus on delivering quality care. When providers are supported and set up for success, it leads to better outcomes and healthier, happier lives for the members we serve across Michigan.
What motivates you in your work?
I’m motivated by the opportunity to solve problems and make a meaningful impact for both providers and members. I take pride in being a trusted, reliable support system and working through challenges head-on to find solutions that benefit everyone involved. I’m driven by collaboration, open communication, and reaching resolutions that strengthen relationships and improve outcomes. Knowing that my work helps providers do their best work, and ultimately improves the quality of care for members, keeps me focused and motivated every day.
